Curacion transmural como objetivo terapeutico en la enfermedad de Crohn

Abstract

Hipótesis y objetivos. La curación transmural puede ir asociada a un mejor curso evolutivo en pacientes con EC, con buena relación con la remisión clínica y biológica. El objetivo de este estudio es analizar el valor de la ecografía abdominal en la evaluación de la gravedad y actividad clínica en los pacientes con EC. Material y métodos. Estudio retrospectivo de una cohorte de pacientes con EC en seguimiento por la unidad de EII del Hospital de Manises entre 2016 y 2020 incluidos en la base de datos ENEIDA. Analizamos variables clínicas (Harvey), biológicas (PCR y CF) y ecográficas (basal y a los 12 meses). Realizamos un análisis univariable y bivariable (Prueba Chi-cuadrado de Pearson y t-Student) a través del programa estadístico SPSS. Resultados. Se incluyeron 36 pacientes con EC. Se observó un porcentaje de reducción entre el valor medio de la PCR basal y al final de seguimiento del 20%, mientras que en el de la CF resultó ser del 31%. Respecto a las variables ecográficas globales, se observó una reducción al final del seguimiento del 8,5% y del 44% en el grosor de la pared y el Doppler color, respectivamente. Conclusiones. El grosor de la pared intestinal y el Doppler color son los parámetros que mejor definen los cambios transmurales a nivel intestinal en pacientes con EC. No hemos encontrado correlación significativa entre los parámetros ecográficos basales y al final del seguimiento con la gravedad clínica (Harvey) y la actividad biológica (PCR y CF).

The aim of this study is to analyse the value of abdominal ultrasound in the assessment of severity and clinical activity in CD patients. Material and methods: Retrospective study of a cohort of patients with CD under follow-up by the IBD unit of the Hospital de Manises between 2016 and 2020 included in the ENEIDA database. We analysed clinical (Harvey), biological (CRP and CF) and ultrasound (baseline and at 12 months) variables. We performed a univariate and bivariate analysis (Pearson's Chi-square test and Student's t-test) using the SPSS statistical software. Results: 36 patients with CD were included. A percentage reduction between the mean baseline CRP value and end of follow-up of 20% was observed, while the mean CF value was 31%. Regarding global ultrasound variables, a reduction at end of follow-up of 8.5% and 44% was observed for wall thickness and colour Doppler, respectively. Conclusions: Intestinal wall thickness and colour Doppler are the parameters that best define transmural changes at the intestinal level in patients with CD. We found no significant correlation between baseline and end-of-follow-up ultrasound parameters with clinical severity (Harvey) and biological activity (CRP and CF).
