Predictibilidad de los movimientos ortodóncicos con alineadores transparentes. Una revisión sistemática

Abstract

Fundamentos: En los últimos años, un número creciente de pacientes adultos han buscado tratamiento de ortodoncia y han expresado su deseo de alternativas estéticas y cómodas a los aparatos fijos convencionales. Desde entonces, el interés y la difusión de esta alternativa terapéutica a la clásica terapia de ortodoncia multibracket ha aumentado exponencialmente, considerando también las necesidades y percepción estética de los pacientes. Los alineadores transparentes pueden producir resultados clínicamente aceptables que podrían ser comparables a la terapia con aparatos fijos. Objetivos: El objetivo de esta revisión sistemática es determinar la predictibilidad de los movimientos ortodóncicos en la terapia con alineadores transparentes. Material y métodos: Se realiza una revisión sistemática de estudios cuyo objetivo principal sea analizar la predictibilidad de los diferentes movimientos ortodóncicos que producen de los alineadores transparentes. La búsqueda bibliográfica se realiza a través de Medline (Pubmed), MedlinePlus, Google Scholar y Scielo. Partiendo de 8963 registros, se examina el título, el resumen y los objetivos de cada uno conforme a los criterios, y se analiza si cumplen los criterios de elección, obteniendo así 114 artículos completos evaluados por criterios. Resultados: En total se incluyen 14 artículos de los cuales 3 versan sobre la predictibilidad del movimiento de distalización, 4 sobre la predictibilidad del movimiento de expansión, 4 sobre la predictibilidad del movimiento de intrusión, 2 sobre la predictibilidad del movimiento de mesialización y 4 sobre la predictibilidad del movimiento de rotación, de los cuales 3 artículos ya se habían usado previamente para el análisis de los otros movimientos nombrados anteriormente. Conclusiones: La predictibilidad de movimiento de la terapia ortodóncica con alineadores transparentes se muestra favorable en los movimientos de distalización molar, expansión de ambos maxilares e intrusión de dientes del sector anterior. La combinación con aditamentos, elementos auxiliares o la técnica de IPR incrementan exponencialmente la predictibilidad de los movimientos ortodóncicos y garantizan así un mayor éxito de tratamiento.

Background: In recent years, an increasing number of adult patients have sought orthodontic treatment and have expressed a desire for aesthetic and comfortable alternatives to conventional fixed appliances. Since then, the interest and diffusion of this therapeutic alternative to the classic multibracket orthodontic therapy has increased exponentially, also considering the needs and aesthetic perception of patients. Clear aligners can produce clinically acceptable results that may be comparable to fixed appliance therapy. Objective: The objective of this systematic review is to determine the predictability of orthodontic movements in therapy with transparent aligners. Methods: A systematic review of studies whose main objective is to analyze the predictability of the different orthodontic movements produced by transparent aligners is carried out. The bibliographic search is carried out through Medline (Pubmed), MedlinePlus, Google Scholar and Scielo. Starting from 8963 records, the title, abstract and objectives of each one is examined according to the criteria, and it is analyzed if they meet the selection criteria, thus obtaining 114 complete articles evaluated by criteria. Results: A total of 14 articles are included, of which 3 deal with the predictability of the distalization movement, 4 on the predictability of the expansion movement, 4 on the predictability of the intrusion movement, 2 on the predictability of the mesialization movement and 4 on the predictability of the rotation movement, of which 3 articles had already been used previously for the analysis of the other movements named above. Conclusions: The predictability of movement of orthodontic therapy with transparent aligners is shown to be favorable in molar distalization movements, expansion of both maxilars and intrusion of teeth in the anterior sector. The combination with attachments, auxiliary elements or the IPR technique exponentially increase the predictability of orthodontic movements and thus guarantee greater treatment success.
